What is the solution for Problem of persistent severe Endodontic pain ?
(J Endod 2009;35:958–961)
study aimed to determine the characteristics of patients with neuropathic tooth pain (NTP) who were selected from a group of patients who developed persistent pain after undergoing endodontic procedures.
Methods: Of 271 patients who had chronic persistent pain that did not respond to previous
endodontic procedures and were referred to the Endodontic Team of the Nippon Dental University Hospital, 16 patients (5.9%; mean age, 46.8 years; 13 women) who fulfilled the diagnostic criteria for NTP were recruited. The inclusion criteria for the patients were the presence of chronic persistent pain and other pain-related symptoms, despite the absence of major pathology. Results: Pain predominantly occurred in the maxilla (14 patients). In 10 patients (62.5%), NTP developed after retreatment. Daily application of tricyclic antidepressants produced pain relief in 11 patients (68.8%).
Conclusions: These results indicated that NTP is a rare type of chronic intractable endodontic
pain and that careful diagnosis of NTP is important.
Surgically Facilitated Orthodontic Therapy: A New Tool for Optimal Interdisciplinary Results
Richard D. Roblee, DDS, MS; Scotty L. Bolding, DDS, MS; and Jason M. Landers, DDS, MS from Compendiumlive
Restorative-driven interdisciplinary therapy frequently requires orthodontic therapy to optimally address the underlying dentofacial problems. Limitations in traditional orthodontic techniques and the length of requisite treatment times often create barriers to providers and patients’ willingness to accept orthodontics as part of the overall treatment plan. Two main types of surgically facilitated orthodontic therapy (SFOT), each with its own indications and protocols, are becoming popular. They can expand significantly the limitations of orthodontics and greatly shorten treatment times. The first uses corticotomies to cause bone demineralization through a regional acceleratory phenomenon (RAP) that “supercharges” dentoalveolar tooth movement. The second type of SFOT involves single- or multiple-tooth osteotomies combined with the principles of distraction osteogenesis to rapidly grow hard and soft tissues, thereby enabling changes in alveoloskeletal relationships. With these SFOT procedures, an interdisciplinary team can modify predictably the dentoalveolar complex so the teeth, alveolar bone, and skeletal components are addressed properly to maximize ideal functional and esthetic relationships, while greatly reducing treatment time and increasing stability of the result. And Now Bluetooth technology for prevention of dental caries
Medical Hypothesis July 2009
Caries is caused when the pH at the tooth surface drops below 5.5. A miniaturized and autonomous pH monitoring nodes can be attached to the tooth surface, like a tooth jewel. This intelligent sensor includes three components: (a) digital micro pH meter, (b) power supply, (c) wireless communicating device. The micro pH meter facilitates long term tooth surface pH monitoring and providing real time feedback to the patients and dental experts. Power supply of this system will be microfabricated biocatalytic fuel cell (enzymatic micro-battery) using organic compounds (e.g. formate or glucose) as the fuel to generate electricity. When micro pH meter detects the pH lower than 5.5, wireless Bluetooth device sends a caution (e.g. “you are at risk of dental caries”) to external monitoring equipment such as mobile phone or a hands-free heads. After reception of the caution, subjects should use routine brushing and flossing procedure or use a medicated chewing gum (e.g. chlorhexidine containing chewing gum) or rinse with a mouthwash.
Endodontics Retreatment, how to succeed ?
Root canal treatments fail because of inadequate cleaning and debridement of the apical third. There are chiefly two options when a RCT fails. You either do apicoectomy (root end resection) or retreat. Apicoectomy cleans and seals apical end, LSX achieves the same objective by non surgical means. A recent review published in JOE July 2009 has clearly concluded that non surgical retreatment is better than apical surgery in long term, see the para below for details.
LightSpeed LSX is very useful in retreatments. Removal of infected area through thorough cleaning is critical for healing after retreatment and LSX instruments alone can do that. Spade shape cutting end of LSX is very efficient in removal of gutta percha as well. And you will have to do almost no retreatment if LSX is used in the first time itself! Non Surgical Retreatment is better than apical surgery in long term?
Journal of Endodontics, Volume 35, Issue 7, July 2009, Pages 930-937
The purpose of this systematic review was to compare the clinical and radiographic outcomes of nonsurgical retreatment with those of endodontic surgery to determine which modality offers more favorable outcomes.
Results
A significantly higher success rate was found for endodontic surgery at 2–4 years (77.8%) compared with nonsurgical retreatment for the same follow-up period (70.9%; P < .05). At 4–6 years, however, this relationship was reversed, with nonsurgical retreatment showing a higher success rate of 83.0% compared with 71.8% for endodontic surgery (P < .05). Insufficient numbers of articles were available to make comparisons after 6 years of follow-up period. Endodontic surgery studies showed a statistically significant decrease in success with each increasing follow-up interval (P < .05). The weighted success for 2–4 years was 77.8%, which declined at 4–6 years to 71.8% and further declined at 6+ years to 62.9% (P < .05). Conversely, the nonsurgical retreatment success rates demonstrated a statistically significant increase in weighted success from 2-4 years (70.9%) to 4–6 years (83.0%; P < .05). Conclusions
On the basis of these results it appears that endodontic surgery offers more favorable initial success, but nonsurgical retreatment offers a more favorable long-term outcome.

Dentist in USA Arrested
Authorities arrested Rivera and his two female dental assistants Monday for practicing dentistry without a license.
Now former patients are left wondering what to do.
"He would use like the same little stick to put rubber bands on the four of us, so we don't know if he would cleaned it or he used it on other people," said Valdez.
Lopez said she doesn't really feel taken advantage of: she and others were looking for cheap treatment. More
Rheumatoid arthritis is linked to oral bacteria: Etiological association
Modern Rheumatology, 07/08/09
This review is to evaluate the association between rheumatoid arthritis (RA) and periodontopathic bacteria. The presented evidence indicates that oral bacteria directly associate with etiopathogenesis of RA. Periodontal infections should be treated and prevented from becoming chronic.
* Clinical studies have provided evidence for a significant association between the 2 disorders
* High levels of oral anaerobic bacterial antibodies have been found in the serum and synovial fluid of RA pts
* Porphyromonas gingivalis, Tannerella forsythensis, and Prevotella intermedia have been identified in RA synovial fluid
* Ornidazole, levofloxacin, and clarithromycin are used in the treatment of infections caused by anaerobic bacteria
* These antibiotics have been shown to be effective against RA
Obesity An Oral Bacterial Disease? ScienceDaily (July 9, 2009) — A scientific team from The Forsyth Institute has discovered new links between certain oral bacteria and obesity. In a recent study, the researchers demonstrated that the salivary bacterial composition of overweight women differs from non-overweight women. This preliminary work may provide clues to interactions between oral bacteria and the pathology of obesity. This research may help investigators learn new avenues for fighting the obesity epidemic. Justice Ravinder Bhat issued the notice after hearing the plea of Sonia Singh, a resident of Nehru Enclave, Kalkaji, and other residents who have opposed the installation of mobile phone towers above their houses.
Seeking removal of the towers, the residents in their plea have argued that radiations coming from the mobile towers are not good for the health of children and pregnant women.
“The generators installed along with these towers create noise, which leads to hypertension among senior citizens,” the petitioners’ counsel M.S. Ahluwalia told the bench.
After hearing the arguments, the court asked the telecom company to respond by July 20 and also directed it to stop installing any more mobile towers till the next hearing.
